POSITION ANNOUNCEMENT/JOB DESCRIPTION
TITLE: Supported Housing Program Manager/Lead Care Coordinator
PROGRAM DESCRIPTION: This position coordinates and supervises the provision of housing and supported services at the Idaho Apartments, Giant Road and LillieMaeJonesPlaza, which are permanent housing facilities for homeless men, women and families who live with HIV, mental health and substance abuse. This position also has primary care coordination responsibilities for the residents of the Idaho Apartments. Responsibilities include case management, staff supervision, and coordinating on and off-site services with third party providers, the community and other stakeholders.
ACCOUNTABILITY: This position reports to the Mental Health & Wellness Senior Program Manager.
A Master’s degree in the social services field; a Bachelor’s degree and additional relevant work experience may substitute for the higher degree. Successful experience planning and implementing client directed services in permanent housing is required; must include experience with HIV, mental health, substance abuse and homelessness;
Solid supervisory experience and solid team leadership abilities;
Interpersonal flexibility, strong teamwork and excellent communication skills;
Computer and internet literacy, including Microsoft Word;
This position requires that the incumbent transport clients as necessary, shop for the program as needed and attend off-site meetings, trainings and other events. Proof of a valid and current driver license, current auto insurance, and an acceptable Motor Vehicle Report are required.
RESPONSIBILITIES (Essential functions include meeting agency productivity standards)
Program Management & Development: Includes developing, implementing, and monitoring program goals and objectives for all programs managed; budgeting; compliance; minimizing housing vacancies, quality assurance and risk mitigation;
People Management & Development: Direct and supervise the staff for all programs managed. Includes resource planning (may include serving as backup to individual program staff in the event of short staffing); program hiring, skills development and performance management;
Compliance, Confidentiality and Quality Assurance: Includes responsibility for all required individual and team documentation & MediCal billing; also includes maintaining confidentiality and compliance with contractual obligations for documentation and data collection;
Care Coordination: (35% of job responsibility) Maintain a caseload up to 14 Idaho Apartment residents; includes assessment, service plan development, service brokerage, coordination and serving as point person for the consumer; services delivered may include vocational assessments and referrals, life skills, housing support and budget development;
Collaboration with Property Management: Includes screening potential new residents for appropriateness for admissions, working with Property Management to facilitate a resident’s smooth transition to other programs in the event that s/he is no longer able to maintain tenancy for health or other reasons; and managing incidents, complaints and dealing with emergencies;
Perform other duties as required.

HOURS: This is a full-time, non-exempt position.
